Virginia Diabetes Council's (VDC) Education and Empowerment Workgroup is pleased to announce the availability of a new tool for children with diabetes in the school setting - Virginia's Diabetes Medical Management School Forms.
The VDC endorses these forms and the Department of Education, Office of Student Services supports their use in all school systems in Virginia. The Diabetes Medical Management Plan School Forms are to be used in conjunction with the Protocols section.
